ACYCLOVIR ANTIVIRAL ACTIVITY ::
The antiviral activity of acyclovir is restricted to the herpes viruses other less sensitive viruses are varicella -zoster virus cytomegalovirus etc it also enhances inhibition of HIV-I infection by zidovudine
It is used in herpes -simplex viral infections It is used systemically by intravenous route (5 mg/kg) as well as topically it is also used in varicella -zoster infections
The adverse effects ::
The adverse effects of topical acyclovir include local irritation and a transient burning when applied to genital lesions intravenous acyclovir is usually tolerated well Rarely it may cause local phlebitis rash diaphoresis nausea sommolence and lethapy hematuria and hypotension it cause netropenia and crystalline nephropathy
Sometimes there may be development of acyclovir resistant herpes simplex causing pneumonia encephalitis and mucocutanceous infections in immunocompromised patients
In high dose valacyclovir may produce confusion hallucinations nephrotoxicity and thrombocytopenia

एसाइक्लोविर की एंटीवायरल गतिविधि हर्पीज वायरस तक सीमित है, अन्य कम संवेदनशील वायरस वैरिसेला-जोस्टर वायरस साइटोमेगालोवायरस आदि हैं, यह ज़िडोवुडिन द्वारा एचआईवी-आई संक्रमण के अवरोध को भी बढ़ाता है
इसका उपयोग हर्पीज-सिम्प्लेक्स वायरल संक्रमण में किया जाता है। इसका उपयोग अंतःशिरा मार्ग (5 मिलीग्राम/किग्रा) द्वारा प्रणालीगत रूप से किया जाता है और साथ ही शीर्ष रूप से भी इसका उपयोग वैरिसेला-जोस्टर संक्रमण में किया जाता है
प्रतिकूल प्रभाव::
स्थानीय एसाइक्लोविर के प्रतिकूल प्रभावों में स्थानीय जलन और जननांग घावों पर लगाने पर क्षणिक जलन शामिल है। अंतःशिरा एसाइक्लोविर आमतौर पर अच्छी तरह से सहन किया जाता है। शायद ही कभी यह स्थानीय फ़्लेबिटिस, दाने, पसीना आना, मतली, तंद्रा और सुस्ती, हेमट्यूरिया और हाइपोटेंशन का कारण बन सकता है। यह नेट्रोपेनिया और क्रिस्टलीय नेफ्रोपैथी का कारण बनता है।
कभी-कभी प्रतिरक्षाविहीन रोगियों में एसाइक्लोविर प्रतिरोधी हर्पीज सिम्प्लेक्स का विकास हो सकता है, जिससे निमोनिया, एन्सेफलाइटिस और म्यूकोक्यूटेनियस संक्रमण हो सकता है। वैलासाइक्लोविर की उच्च खुराक से भ्रम, मतिभ्रम, नेफ्रोटॉक्सिसिटी और थ्रोम्बोसाइटोपेनिया हो सकता है
