Enfuvirtide is the only available HIV entry inhibitor it is 36 amino acid synthetic peptide 

Mechanism of Action ::

        The peptide blocks the interaction between the N36 and C34 sequences of  the gp41 glycoprotein by binding to a hydrophobic groove in the N36 coil This prevents formation of six-helix bundle critical for membrane fusion & viral entry into the host cell it inhibits infection of CD4+cells by free virus particles as well as cell to cell transmission of HIV in vitro 

        HIV can develop resistance to this drug through specific mutations in the enfuvirtide-binding domain of gp41

        Adverse reaction includes a local side effects including pain erythema & induration at the site of injection are seen 

Fomivirsen ::

fomivirsen is 21-base phosphorothioate oligonucleotide It inhibits the viral binding to cells It is active against CMV strains resistant to ganciclovir foscarnet & Cidofovir

क्रिया का तंत्र ::

पेप्टाइड N36 कॉइल में हाइड्रोफोबिक खांचे से बंध कर gp41 ग्लाइकोप्रोटीन के N36 और C34 अनुक्रमों के बीच की बातचीत को रोकता है। यह मेजबान कोशिका में झिल्ली संलयन और वायरल प्रवेश के लिए महत्वपूर्ण छह-हेलिक्स बंडल के गठन को रोकता है। यह मुक्त वायरस कणों द्वारा CD4+ कोशिकाओं के संक्रमण को रोकता है और साथ ही इन विट्रो में एचआईवी के सेल से सेल संचरण को रोकता है।

gp41 के एनफुविर्टाइड-बाइंडिंग डोमेन में विशिष्ट उत्परिवर्तन के माध्यम से एचआईवी इस दवा के लिए प्रतिरोध विकसित कर सकता है

प्रतिकूल प्रतिक्रिया में स्थानीय दुष्प्रभाव शामिल हैं जिसमें दर्द एरिथेमा और इंजेक्शन के स्थान पर कठोरता देखी जाती है।

फोमिविर्सेन ::

फोमिविर्सेन 21-बेस फॉस्फोरोथियोएट ऑलिगोन्यूक्लियोटाइड है। यह कोशिकाओं से वायरल बंधन को रोकता है। यह गैन्सीक्लोविर के प्रतिरोधी सीएमवी उपभेदों के खिलाफ सक्रिय है। फोस्कारनेट और सिडोफोविर
